Step 1: Contact your new clinic and request a transfer of medical records form (some are also linked above for your convenience).
Step 2: Complete the form and sign it.
Step 3: Your new clinic will need to email this request form to our Receptionist, Hannah at hannah.bailey@bettermedical.com.au
Step 4: Hannah will liaise with your new clinic and facilitate the records transfer.
