Please refer to the steps below to have your medical records transferred to your new clinic.
For privacy reasons, patient records cannot be released directly to patients. However, if you would like our friendly reception team to transfer your healthcare records to another practice, please complete a transfer request form (see below).
TO TRANSFER YOUR PATIENT RECORDS
Step 1: Contact your new clinic and request a transfer of medical records form.
Step 2: Complete the form and sign it.
Step 3: Your new clinic will need to email this request form to our team at firstname.lastname@example.org
Step 4: We will liaise with your new clinic and facilitate the records transfer.
